How can I witness someone's decision?
Organ donation decisions must be witnessed.
Role of the witness
Organ donation decisions must be witnessed.
The role of the witness is to verify and declare that the person who appointed them is the one making the decision. The declaration serves as an official acknowledgment of the decision.
Requirements to be a witness
To be a witness, you must:
Be at least 21 years old.
Not be mentally disordered.
Hold a Singpass account (for online submissions only).
Online submission
Has the registrant submitted their decision online?
Log in to the Organ Donor Registry with Singpass to witness it.
How to complete your declaration:
Ensure the registrant shares their intention with you.
You will receive an SMS notifying you to complete your declaration after the registrant submits their decision.
Complete your declaration within 7 days of receiving the notification.
If you fail to do so by this deadline, the registrant's decision will be cancelled automatically. The registrant will have to re-submit their decision subsequently.
Mail submission
If the registrant chooses to mail a hardcopy form, complete the witness section after the registrant has filled in the form.
